Ovarian stimulation treatment

Ovarian stimulation

The aim of ovarian stimulation treatment, i.e. ovulation induction, is to grow ovarian follicles and to ensure releasing of the egg cell. Oral or injected hormonal preparations are used in order to stimulate the ovaries. The collective term FSH injections are used by us to cover these injected medicines.

Registration for treatment and starting the treatment

One always registers for the treatment at the beginning of the menstrual bleeding (1st – 3rd day of the cycle) by calling the specialist nurse during the telephone hours. Instructions for beginning the medication are given over the phone and an appointment is reserved for a follicular sonogram.  

The menstrual cycle: First day of the menstrual cycle = first day of menstrual bleeding. If the bleeding begins in the evening after 18:00, then the following day is considered to be the first day of menstrual bleeding.
Clomifen®,  Tamofen®, Femar® - tablets are used for 5 days at the beginning of the cycle. The date for beginning is agreed patient specifically.  
 
FSH injections (Puregon®, Gonal-F®, Menopur®, Fostinom®) should be administered either in the afternoon or the evening according to an earlier given protocol. Repeated sonograms and measurements of the hormonal level in the blood are part of the treatment follow-ups. The blood test should be taken before 08:00 in order for the results to be available before noon the same day.

  • The laboratory is located on the 12th floor of the U Hospital, and it is open as of 07:00. 

Follicular sonogram:

The examination is performed vaginally, why the urinary bladder has to be empty. Instructions for continued care are given after the examination, either directly at the appointment or later by phone on the specialist nurse’s telephone hours. If an insemination is planned to be combined to the treatment, a jar for semen will be given along for this.

Time of the ovulation:

In order to determine the ovulation time ovulation, i.e. LH tests have to be conducted at home as of an agreed date. The test can be purchased from the pharmacy or a well stocked food store. We do not recommend tests bought from the internet due to their varying quality. When the ovulation test is positive the specialist nurse should be called about this. 

Releasing of the egg cell can also be achieved using a Pregnyl® / Ovitrelle® injection. The timing of the injection is disclosed during an outpatient ward appointment. 
 
Insemination / copulations:

A separate patient instruction is given regarding arrival for insemination. 

If insemination cannot be performed, copulations should be timed to take place every other day when the LH test has become positive or the following couple of days after the releasing injection. The mature egg cell is released about 24 hrs after the LH test becomes positive. The ovulation occurs about 36 hrs after the releasing injection.

The pregnancy test is conducted 18 days after the ovulation test became positive or after the releasing injection.
